I declare that I will be responsible for the following:

Medical and/or any hospital accounts which may occur for the child, in the event of illness or accident where I cannot be consulted in time;

Payment of bank charges

Payment of school fees by or before the Agreed Upon Date of Payment each month. In the case of late payment of my school fees I acknowledge that I will pay a late payment penalty fine of R100 and that a transfer card will be issued upon signing an acknowledgement of debt to the school. In case of non payment of school fees, I acknowledge that GoodHope Mixed College shall be entitled to take legal action to recover the amount outstanding with interest at prime rate, and further that I shall be responsible for any fees arising from such legal action.

To give GoodHope Mixed College one terms paid notice should I decide to transfer my child to another school

We/I as a parent/guardian cede my power as a parent to the Principal of GoodHope Mixed College or his/her representative to consent to emergency medical treatment by any qualified medical practitioner. I acknowledge that the Principal and/or all the educators of GoodHope Mixed College, at all material times and in all respects act in loco parentis regarding education, care and discipline of my child.

We/I indemnify all employees in the service of GoodHope Mixed College College against any claims of any nature arising:

• Out of the conduct and acts of the learner;
• Out of events connected with travelling;
• Out of school activities generally.

We/I waive any claim against GoodHope Mixed College and its employees for anything bona fide done under this authority.
